Community Mental Health Nurse Join our dynamic team as a permanent Community Nursing and Care Coordinator in the vibrant Bethnal Green and Globe Town Neighborhood Mental Health Team! In this position, you will conduct thorough assessments within Health (SAP/CPA) and Social Care (Care Management) frameworks, allowing you to develop, implement, and continually refine tailored care plans. Your expertise will be invaluable in supporting and advising family members and caregivers, ensuring they have the guidance they need. Collaborating closely with crisis pathway services, you'll play a crucial role in delivering a safe and responsive urgent community assessment and treatment service. You’ll conduct comprehensive risk assessments with efficiency and effectiveness, working collaboratively to formulate and agree on crisis safety care plans. Your supportive approach will make a difference to relatives and caregivers during critical times. To assess, plan, set goals with the service user, carers and families; implement and evaluate care using psycho-social interventions and skills to meet individual needs. To actively support the person in crisis in self management of their symptoms through the formation of their care plans. To act as a care coordinator for service users. To be responsible for accurate recordkeeping in line with Trust and NMC policy and procedures. The post holder will contribute to establishing and maintaining liaison with the members of the primary/secondary healthcare team, voluntary agencies in the professional disciplines necessary to provide a comprehensive service to clients and their families Complete assessment and discharge documents in a timely fashion The post holder will have working relationships within a multidisciplinary team and the wider crisis pathway.
